A) Full name and email address of the individual reporting the infringement.B) A hyperlink to the specific page in question.C) The nature of the complaint (copyright infringement, obscene, questionable or abusive content, defamation, mis-information etc...).D) In case of suspected mis-information, either 1) provide a link(s) to the correct source(s) of information, or 2) include, in text, the correct information.E) Confirmation that if you're reporting a copyright infringement, you are, 1) the copyright holder, or 2) You are authorized to act on the copyright holder’s behalf.
